About
Founded by Melissa Traversa, CareConnectSydney is built on a foundation of professional expertise and a deep commitment to inclusive,
person-centred care.
With a background in nursing, early childhood inclusion support, and parent education –including certification as a Triple P Parenting Program provider, Melissa brings both clinical insight and empathy to her work. She now leads a dedicated team supporting individuals and families from early childhood through to adulthood.
We provide Support Coordination and tailored guidance for participants aged 0–40 years, with a particular interest in supporting children with complex medical and disability needs. Our work combines clinical understanding with practical systems navigation, offering informed, capacity-building support, particularly for women and families who have felt overwhelmed in their NDIS journey.
In addition, we partner with early childhood communities to strengthen inclusive practice and foster collaborative, sustainable support networks.
